Leadership sees one picture. The people doing the work see another. One department experiences a problem while another may be making perfectly reasonable decisions that contribute to it. Technology, processes, expectations, and ways of working all affect each other, but we tend to look at them separately.
So the problem gets named from where it shows up. A system gets blamed for confusion that was already there. Departments get frustrated with each other without seeing what each is responding to. Leaders ask for better data when the real challenge is making sense of information scattered across disconnected places.
That means people can be right about what they're seeing and still be missing part of the picture.
The problem you can see may be real. It just may not be the whole problem. And when you solve only where the problem shows up, the underlying conditions can follow you into whatever you do next.
You've likely paid for this problem once already. The question is whether you'll pay for it again before you actually see it.
